Clinical evaluation of three long-acting anticholinergic compounds.

چکیده
Anticholinergic drugs are used widely in the treatment of peptic ulcer, though their value is disputed. The ideal preparation should be long-acting, and should reduce gastric acid secretion throughout both day and night without causing side effects. Short-acting preparations were shown to inhibit gastric secretion only in doses which produced side effects (Sun and Shay, 1955). Different conclusions were reached by Mitchell, Hunt, and Grossman (1962), who found that both atropine and poldine, a longer-acting synthetic compound, inhibited gastric secretion without causing side effects. Such disagreement may in part be due to differences in methods of assessing anticholinergic agents. But doubt remains as to whether side effects are a necessary accompaniment to the effective treatment of peptic ulcer with anticholinergic drugs. In this report, we have examined some of the available long-acting preparations. Measurement of salivary flow has been used to assess their duration of action. We have also compared the effects of the different preparations on visual accommodation, salivary flow, and gastric acid secretion.

The role of long-acting bronchodilators in the management of stable COPD.
Bronchodilators form the foundation of symptomatic treatment of COPD. Several long-acting bronchodilators are now available for use in COPD, but publications of large-scale studies of their efficacy have, for the most part, postdated the publication of major clinical guidelines. This article provides a critical review of large (> or =50 patients), double-blind, clinical trials of three long-act...

The acute intravenous toxicities of a selection of three potential long-acting local anaesthetics belonging to the series of2- phenoxyethyldialkylamine were determined in mice. Two of the compounds were tertiary amines and the third a quaternary ammonium derivative. The I.V. median lethal dose, (LDso) of one of the tertiary compounds, 21A [N, N- diethyl-2- (2,6- diisopropyl) ethyl amine hyd...
متن کاملTrospium chloride: an update on a quaternary anticholinergic for treatment of urge urinary incontinence
Trospium chloride is a quaternary ammonium compound, which is a competitive antagonist at muscarinic cholinergic receptors. Preclinical studies using porcine and human detrusor muscle strips demonstrated that trospium chloride was many-fold more potent than oxybutynin and tolterodine in inhibiting contractile responses to carbachol and electrical stimulation. The drug is poorly bioavailable ora...

متن کاملSystematic Review and Meta-analysis of Anticholinergic Side Effects of Long-acting Antipsychotics
Background: There are few studies on the anticholinergic side effects of long-acting antipsychotics. They tend to be used with stigmatized, severely ill and non-concordant patients rather than first episode psychosis. Aim: To investigate prevalence/incidence rate of anticholinergic side effects of long-acting antipsychotics. Methods: We included all participants with schizophrenia, schizoaffect...
